Diabetes – Take Action

Take Action is our 8-week group-based program combining education and exercise designed to empower you to take back control of your health and assist you with the knowledge to manage your diabetes. This program is for all individuals living with diabetes including pre-diabetes, Type 1 and Type 2.

The program is designed to provide you with the skills and the knowledge needed to improve glucose management, increase fitness and function, maintain a healthy body weight and improve your lifestyle risk factors.

The Take Action program is facilitated by one of our Exercise Physiologists who will work with you over the course of the 8 weeks to have a better understanding of monitoring your glucose levels and how to safely exercise with diabetes. Throughout the 8 weeks you will have the opportunity to connect with other individuals living with diabetes in a friendly and supportive environment all while learning how to better self-manage your diabetes.

  • 1:1 initial assessment to establish baseline measurements of current health and physical capacity as well as setting out your goals for the program.
  • 2 x Group exercise classes per week for 8 weeks
  • Weekly short education session to increase your knowledge around living with diabetes and managing your risk factors.
  • 1:1 final assessment to evaluate your progress and provide you with individualised guidance on your plan moving forward.
  • A broad knowledge of how to safely mange diabetes with exercise
  • Individually prescribed exercise program to assist you in independently managing your diabetes
  • Improved management of you blood glucose levels and blood pressure and the skills to self-monitor
  • Access to comprehensive resources to help you build your good habits beyond the 8 weeks of the program
  • Reduction in diabetes related complications e.g., stroke, heart attack, kidney disease, foot, and eye problems)
  • Improved mood, sleep quality and energy levels
  • Improved strength, endurance and exercise tolerance

Yes, those individuals diagnosed with type 2 diabetes can request their GP to include the Diabetes Group Referral as part of their Enhanced Primary Care Plan using the Diabetes Program Referral form (link to form). This referral can be used to claim a subsidised initial assessment as well as one subsidised session each week over the course of the 8 weeks.
